Source: ORL. Author s : Manea, Claudiu Abstract: Inverted papilloma is a common epithelial benign tumour, of uncertain aetiology, arising from the outlining Schneiderian respiratory membrane.
This locally aggressive neoplasm is a lesion of the mucosal membrane of the nasal cavity and the paranasal sinuses, composed of welldifferentiated columnar or ciliated respiratory epithelium. It has a high risk of association with malignancies.
The endoscopic approach was performed through a small bone window by the midline of the glabella, with inverted papilloma in nasal cavity restoration of the bone support at the end of the surgery using a titanium mesh. Although the surgical staff has a lot of experience in doing the endoscopic transnasal approach, there are some cases where the transnasal route cannot safely solve certain pathologies. This is the reason why the authors wish to illustrate the need of using an approach that will ensure a proper management of any kind of complications that can occur during surgery, specific to these tumors bleeding, cerebrospinal fluid leak. In this case, the transcranial endoscopic approach was the best solution. Surgery is the primary treatment. After surgery, the risk of local recurrence varies with the extent of resection, with an often recurrence if incompletely excised.
We conclude that the endoscopic inverted papilloma in nasal cavity has proven to become a safe and effective method over the traditional approach. Nevertheless, in cases of advanced tumour stages, or malignancy, the best surgical option is to use a wide access, as radical antrostomy or paralateronasal rhinotomy. English Copyright of ORL.
